By completing this registration form I understand and agree to the following:

- Upholding a standard of conduct that reflects the Christian values of our organization and respects the communities we serve. Participants are expected to act with integrity, humility, and a spirit of teamwork at all times. The use of alcohol, illegal drugs, or any substances that impair judgment is strictly prohibited throughout the duration of the trip. We ask that all team members commit to being positive representatives of GO on the Mission and support a safe, respectful, and mission-focused environment.

- All trip costs will be handled by Immanuel Lutheran Church.
Half payments will be due October 1st and Final Payments will be due December 1st. Amanda Berg, the group leader, handle all balances and payment information.

- There will be a series of 3-4 trainings prior to the trip (dates TBD) it is crucial that every attempt is made to be present. However, if missed it is understood you will find a time to meet with the trip lead to go over missed information.

- GO on the Mission requires several Release Forms which will be filled out and notarized at a team planning meeting. These forms will be given to you ahead of time to review.
